Food & Nutrition Service (FNS) School Meal Program Complaints


Hoffman Homes will receive and process complaints alleging discrimination within the school Meal program. An allegation may be made verbally or in person to the VP of Safety & Facilities Management. The receiver will transcribe the complaint. The receiver will supply a USDA Discrimination Complaint Form (control number 0508-0002) to the complainant The receiver (SFA) will not prevent a complaint from being received, accepted or filed. Complaints cannot be attempted to be resolved internally by the receiver of the complaint. The above process, as stated, will not be held as a prerequisite for a complaint to be filed. The SFA must designate and file the complaint with one of the two agencies below:

School Breakfast Program (SBP) and Summer Food Service Program Outreach
Hoffman Homes, as a School Food Authority (SFA), publicizes the availability of the School Breakfast Program before the school year and during the school year via our website and also at the time of student admissions into the RCCI program.


The SFA also makes available, for eligible families, free meals under the Summer Food Service Program. You can locate sites that serve free meals to children during the summer, in any of the following ways:
  • Call 211
  • Call 1-866-3Hungry or 1-877-8Hambre
  • Text “MEALPA” to 877877 during the summer months
