Transitional Living Program

Hoffman Homes for Youth is pleased to now offer a Transitional Living Program (TLP) for females ages 16-21. The TLP will provide skill development and support to effectively transition youth and young adults into the community setting.

For more than 110 years, we have offered hope to youth/young adults and families in Pennsylvania. The Transitional Living Program allows us to extend the same feelings of hope and peace to older youth and young adults while guiding them to thrive in their local communities.

The services provided by the Transitional Living Program include:

  • Individual, Group, and Family Therapies*
  • Care, supervisions, and support by qualified staff
  • Creative therapies that include equine assisted, art, and animal-assisted therapy
  • Education services, either on/off-site for grades 1-12
  • Vocational training/education on-site and in the community
  • Life Skills training/education
  • Medical services provided on-site
  • Community integration opportunities

*When applicable, therapies will be provided through an outpatient provider. Hoffman Homes for Youth will assist the teen/young adult in identifying a provider.
